NEW RULES
- Arrive no less than 15 minutes before your appointment
PVMED has new paperwork requirements due to our new EHR beginning 2021.
If you have not completed our NEW (2021) paperwork, please arrive 15 minutes before your appointment.
If your paper work is current, please arrive 10 minutes before your appointment and
STAY IN YOUR CAR if you arrive too early.
- Please do not arrive late for your appointment
- YOU MAY NOT bring additional family members to your appointment
Family members must wait in your car
Parents of minors and patients who require physical assistance are an exception
- Please wear a mask at all times
- Come into office, notify reception, DO NOT sit down
A member of the nursing staff will be with you immediately
Telephone questions you will be asked before your appointment
Have you or anyone in your family (household) tested positive for COVID-19 in the past 14 days?
Have you or anyone in the family (household) been tested for COVID-19 and are awaiting results?
Have you or anyone in the family (household) recently lost your sense of smell or taste?
Do you or anyone in the family (household) currently have any of the following respiratory symptoms?
Fever? Sore Throat? Cough? Shortness of Breath?
Do you or anyone in the family (household) have any GI symptoms? Diarrhea? Nausea? Vomiting?
Even if you do not currently have any symptoms,
Have you or anyone in the family (household) experienced any of these symptoms in the last 14 days?
Have you or anyone in the family (household) been in contact with someone
who has tested positive for COVID-19 in the last 14 days?
Have you anyone in the family (household) traveled anywhere outside the country in the past two weeks? If answered “yes” to the above question,
where did the patient or anyone in the family (household) travel?
If your answer is YES to any of the above questions, your appointment might be cancelled.
We are sorry, but given the potential risk of exposing other patients and ourselves, we may cancel any patient appointment suspected to have COVID-19 symptoms. A nurse will speak to you, to discuss your next steps.
